This is the fourth of four Halloween inspired mandala charts. These were originally published in the Just Cross Stitch Halloween Books, and are now being made available as stand-alone leaflets.
It's shown stitched in "London Fog" Sullivans thread on 32 count Attic Lace linen by Lakeside Linens. This one would look great stitched on Glow-In-The-Dark fabric, too. Stitch count is 115 square.
